Pill Conversion: Forces good pills to convert into their negative counterparts. How the Stat Buffs Work

While it forces negative pills, taking those “bad” pills grants powerful secondary rewards:

Stat-Down Pills: Taking any pill that reduces a stat (e.g., Tears Down, Speed Down, Range Down) grants a permanent Damage Up bonus alongside the stat penalty.

Non-Stat Pills: Taking a negative or chaotic pill that does not affect raw stats (e.g., Amnesia, R U A Wizard?, Addicted) spawns a Black Heart on the floor instead.

Neutral Exceptions: Purely neutral cosmetic pills like I Found Pills or Relax offer no secondary bonuses or hearts. Notable Item Synergies

PHD / Lucky Foot / Virgo: If you acquire these items alongside False PHD, pills can roll as both positive and negative again. Taking a bad pill will still successfully grant the False PHD damage or heart bonuses.

Placebo: Allows you to repeatedly copy a specific bad pill to continuously farm damage upgrades or black hearts on command.

Rock Bottom: Prevents your stats from ever dropping, allowing you to gain the raw Damage Up bonuses from stat-down pills without suffering any of the stat penalties.
